So when I created my own campaign setting, I just created a big Infernus that included all these beings (demons, devils, daemons). The layers intertwine like a helix and you can use the Styx, as well as other gates, to travel between layers. Arduin was full of demons and hells. I would not be surprised to learn that people who claimed D&D was demonic or cultic were reacting more to Hargrave than Gygax. I distinctly remember him spinning tales of an earlier D&D group he'd been in that was united in a love of evil, spell-casting, demon-worship, and double-dealing.
